Estos formularios deben ser completados y revisados antes de ser aceptados en Children's Flight of Hope. Además, estos formularios serán actualizados anualmente por todas las familias que regresan.
Formulario de aprobación de transporte aéreo - OBLIGATORIO
Children's Flight of Hope verifica que cada niño esté médicamente autorizado para volar en un avión comercial antes de proporcionar asistencia de vuelo.

Family Financial Eligibility Form- REQUIRED
This form is used for our organization to gather data and information so that we can better serve our families. This form requires a client ID to complete. You will receive your Client ID through email from the missions team once you have submitted a new application.
Waiver- REQUIRED
CFOH requires this form to be completed. This form requires a client ID to complete. You will receive your Client ID through email from the missions team once you have submitted a new application.
Urgent Medical Appointment Form – REQUIRED ONLY IF TRAVEL IS LESS THAN 14 DAYS AWAY
(Returning Families Only)
CFOH requires that flight requests be submitted at least 14 days prior to departure. This requirement applies to every flight request for both new and returning clients. If circumstances require less than 14-day notice, this form is required for returning families only. CFOH is unable to assist new families with flight requests submitted less than 14 days before departure.Completing this form does not automatically approve this request for exception.
*This policy does not apply to a rebooking of a flight due to airline cancellations or extended hospital stays.This form will only be accepted if signed by the child’s medical doctor, case manager, social worker, or another relevant professional on the child’s medical team. Requests without proper certification will NOT be processed.
